If you did that, that’s a total turn-off as that person wouldn’t be considered for admission by the school.

Failure to Upload O’ level results when needed

Your O’ level result makes up a percentage of your stance of gaining admission, you might have uploaded it before, but you might still need to check again to confirm if it’s still there, there have been times when aspirants uploaded their results earlier and it didn’t reflect, you have to re-upload the results.
